Commercial drivers in Texas used Form CDL-7, Commercial Driver License Self-Certification Affidavit, to tell the Department of Public Safety what type of commercial operation they perform and whether they must keep a DOT medical certificate on file. This form helped DPS match each CDL holder’s driving category with the correct medical certification rules and protect drivers from unwanted license downgrades.
Form CDL-7 collected the following information:

✔️ One of four operation categories (non-excepted/excepted, interstate or intrastate).
✔️ Medical examiner’s certificate details and expiration date when a medical card was required.
✔️ Driver’s signature confirming that the self-certification category selected was accurate.
It supported CDL issuance, renewal, and compliance with federal medical-certification rules.
Note: The Texas DPS has phased out Form CDL-7. For medical certification and self-certification, drivers can now use Forms CDL-4, CDL-5, and CDL-10.
