1.Pl. pay registration charges amounting to Rs. 2000.00 by cash or DD favouring UGVCL payable at Mehsana
2.Copy of last regular monthly energy bill.

4.Certified copy of Memorandum of Articles of incoming party.
5.Attested true copy of Registered partnership deed.
6.Documentary evidence to show the dissolution of the old partnership and settlement with the outgoing partners.

8.Undertaking duly notarized on a stamp paper of Rs.100/- as per the attached draft to bear the liabilities arise out of the agreement executed by the outgoing consumer with the Board from incoming party.
