Completing this power of attorney form revokes all earlier chosen POAs and shall be effective until the DOR receives the revocation in writing. The paper loses validity after 12 months.
It is necessary to provide the Attorney-in-Fact’s valid driving license or the state of Georgia ID card when submitting the form. Only an original version of the document will be accepted.

There is a set of simple instructions developed to help you fill out the form successfully:
1. Appoint the Agent
The vehicle owner should print or type their full legal name and provide the agent’s full legal name afterward. Only one Attorney-in-Fact may be chosen to represent the owner.
2. Identify the Vehicle
You will need to enter the VIN, year, make, and model of the motor vehicle.
3. Sign the Paper
Indicate the date of the agreement (month, date, and year), the owner’s full legal name, mailing address, phone number, and affix the signature, claiming to convey the vehicle-related rights to the chosen Attorney-in-Fact willfully.
4. Notarize the Form
A licensed notary has to verify the submitted data and authenticate the VPOA. The notary may ask the owner to show a copy of the driver’s license and input the information concerning the license herein. The date when their commission expires has to be indicated as well.
