Medical power of attorney acts as the document in which you appoint your agent for medical issues. The agent will be able to speak for you if you are unable to communicate with healthcare providers on your own. Be sure to make sure that the designated person is aware of your wishes. Iowa law requires a durable power of attorney for health care to be dated and either signed by at least two witnesses who saw you sign it, in your presence and each other's, or acknowledged before a notarial officer in Iowa.[4]
