Identify the Child
The parent or the appointed guardian is supposed to enter the child’s name and date of birth.
Introduce the Principal
Submit details of the child’s parents or the guardian appointed by the court. You need to state how you’re related to the child by ticking the corresponding box.
The section requires you to enter the legal names and physical address(es) of both lawful parents or legal guardian(s).
Introduce the Attorney-in-Fact

Provide Affirmation by Witnesses
The Missouri POA for a minor child demands that you sign the document in the presence of two witnesses who will affirm the procedure by submitting their names, signatures, and the current date.
Ensure a Notary Public Certification
The final part is obtaining authorization of a notary public who will acknowledge the involved parties’ identities and affix his seal and signature as validation.
Completing this section and acquiring notarization is essential; otherwise, the form is incomplete and null.
