USCIS Form I-134, Affidavit of Support, is a document used to show that visa applicants have sponsorship and will not become public charges while in the United States. This form is commonly used for non-immigrant visa applications, including tourist, student, and fiancé visas, among others. It provides a financial guarantee to the U.S. government from a sponsor, usually a friend or relative, who agrees to financially support the visa applicant during their stay in the U.S.

The form requires the sponsor to disclose their financial information, including income, assets, and liabilities, to demonstrate their ability to support the visa applicant adequately. The sponsor’s commitment is not legally binding like the Affidavit of Support Form I-864 used for immigrant visa applicants, but it still serves as an important document in the consular officer’s decision-making process regarding the applicant’s visa approval. It reassures the U.S. government that the visa applicant will not rely on government assistance during their visit.

  • Identify Yourself

Check the appropriate box denoting your status in the United States.

  • Finish the “Information About You” Section

Here, state your age and how long you have resided in the United States.

  • Provide Information About the Beneficiary

Include the names, dates of birth, gender, country of citizenship, and marital status of the beneficiary. Also, state their A-Number if they have one. Finish the section by specifying their relationship to you (the sponsor).

This is a standard section of the form where you should state their full physical address.

Here, state your annual income, savings account balance, the value of your personal property, and the market value of your stocks and bonds. Also, say if you have life insurance, how much it is, and the cash surrender value.

If you own real estate or have a mortgage, state the value and its address here.

Finish “Part 6” by signing it if you are the preparer. Include the date of signing.

  • Provide Additional Information (If Applicable)

“Part 7” of the form may be left untouched unless there is information that has not been included in the earlier sections of the form because of the lack of space. Identify the page, part, and question numbers to which your answers refer.
