The ATF form 10 is an application for permission to make or import a firearm. The form must be completed by the individual making or importing the firearm, and must include themaker's name and address, the type of firearm being made or imported, and the reason for making or importing the firearm. The form also requires a description ofthe firearm, as well as either a photograph or a detailed diagram of the weapon. Completed forms must be sent to the ATF National Firearms Act Branch inWashington, D.C. for approval before any firearms may be created or imported. Not submitting this form may result in criminal prosecution.
