Congressional Record Book Form PDF Details

The Congressional Award Record Book is the tracking document used by participants in the Congressional Award program, the only award established by the U.S. Congress to recognize voluntary public service and personal achievement in young Americans. Participants ages 13.5 to 23 use this record book to log hours across four activity areas: Community Service, Personal Development, Physical Fitness, and Expedition/Exploration.

Medal Levels and Hour Requirements

Each entry in the record book must include the participant's name, supervising adult information, goal start date, and completion date. All entries are verified by a qualified supervisor and reviewed by a local Congressional Award council before a medal is issued. For supplemental documentation of volunteer hours, the community service form provides additional support.
