Cy47 Form PDF Details

In the world of emergency responders, the responsibility of protecting the most vulnerable extends beyond providing medical care. The CY 47 form represents a crucial tool in this protective measure against child abuse and neglect. Designed specifically for EMS providers, this form serves as a mandatory report that must be submitted within 48 hours after an oral report is made to ChildLine, Pennsylvania's statewide hotline for reporting suspected child abuse. Completing the CY 47 requires diligence and attention to detail, as EMS providers might not have extensive information about the family in question. Nevertheless, they are required to fill out the form to the best of their ability. It's also a practice not unique to EMS providers; other mandated reporters, such as emergency room nurses, are also required to submit a separate CY 47 form if they report suspected abuse concerning the same family. The form demands the reporter's contact information for possible follow-up queries by the Children and Youth investigator, ensuring this information remains confidential and is only shared with law enforcement if necessary. The guidelines stress the importance of a timely oral report to ChildLine, followed by the submission of the CY 47 form to the appropriate county Children and Youth agency. Additionally, the protocol allows for follow-ups by the mandated reporter if there has been no feedback within 30 days, enabling them to inquire about the investigation's outcome and any services provided to the family in question. This comprehensive approach underscores the collective effort required from various sectors to safeguard children from harm.

The CY 47 Form

As mandated reporters of suspected child abuse and neglect, EMS providers are required to submit a CY 47 form to the county Children and Youth agency within 48 hours of making the oral report to the statewide hotline, called ChildLine. Please complete as much as possible on the CY 47. You may have very little information on the family but you are still required to complete what you can and submit the form. If another mandated reporter i.e. emergency room nurse also makes a report of suspected abuse to ChildLine on the same family, that person will also complete and submit a separate CY 47 form. It is important to list your name and phone number on the CY 47 if the Children and Youth investigator has additional questions for you. This contact information can not be shared with anyone except if law enforcement must be notified; then the reporting source is treated as a confidential informant. Additional CY 47 forms are available by contacting ChildLine.

Follow the basic guidelines in reporting suspected child abuse and neglect:

Call ChildLine as soon as possible with the oral report 1-800-932-0313

Complete a CY 47 and mail to the county Children and Youth agency

Follow Up
