Standards: character, presence, intellect, leads, develops and achieves are each marked far exceeded, exceeded, met or did not meet standards, or not evaluated.
Overall achievement: Commandant's List (limited to 20%), Superior Academic Achievement (21% to 40%), Achieved Course Standards or Failed to Achieve Course Standards.
Comments required: for a "Failed" APFT, a "Profile" when it precludes performance of duty, and "No" for Army Weight Standards.
