What it is: This Individual Jump Record will be a permanent document. Jumps will be entered only upon presentation of DA Form 1306 as required by AR 600-8-22.

How to fill out DA Form 1307, box by box

Name and grade
Box 1, NAME: enter Last, First, Middle Initial. Box 2, GRADE: enter the grade.
Qualified as
Box 3, QUALIFIED AS, has the lines a. PRCHST, b. JM SCHOOL GRAD, c. SENIOR PRCHST and d. MASTER PRCHST.
Each of those lines has Box 4, UNIT, Box 5, HEADQUARTERS and Box 6, DATE.
Box 7: one line for each jump (columns a to h)
Box 7, LINE NO.: enter the line number. Box 7a, PLACE OF JUMP: enter the Name of DZ.
Column b, TYPE OF DUTY (JM Duty Only): use AJ/PJ for Jumpmaster INCLUDING Army Aircraft, JMR/(TT) for Jumpmaster Refresher/Transition Training, DACO for Departure Airfield Control Officer, ADACO for Assistant Departure Airfield Control Officer, DZSO for Drop Zone Safety Officer, ADZSO for Assistant Drop Zone Safety Officer and SAF for Safety.
Column c, TYPE OF JUMP: one or more code symbols may be used.
The code symbols are A/NT for Administrative or Non-Tactical, BAR for Basic Airborne Refresher, C for Combat, CE for Combat Equipment, H for Halo, MT for Mass Tactical, N for Night, O for Oxygen, S for Standoff/Haho and T for Tactical.
For example: T - CE - N indicates a 'Tactical' Jump performed with 'Combat Equipment' at 'Night'.
Fill in d. TYPE OF PARACHUTE, e. TYPE OF AIRCRAFT, f. DATE and g. PAY PERIOD COVERED.
h. INITIALS OF PERS. OFFICER: the box for the PERS. OFFICER's initials.
