You have 180 days from the date of delivery to notify the Service Provider in writing of any additional losses or damages discovered after delivery.
At delivery: if there are no missing items or damages discovered at delivery, write "NONE."
Signing: customer and Service Provider must jointly complete and sign this document before the Service Provider leaves your home.
Blank form: the Customer will not, under any circumstances, sign a "blank" or "partially completed" DD Form 1840.
Copies: the Service Provider provides the Customer three (3) copies of the DD 1840 and DD 1840R.
