DD Form 250 at a glance
What it is: DD Form 250 is the Material Inspection and Receiving Report.
Who signs: Enter the date, signature of authorized Government representative, typed name, title, mailing address and commercial telephone number.

How to fill out DD Form 250, box by box
Contract, shipment and invoice, Boxes 1 to 8
Enter the Procurement Instrument Identification (Contract) No. in Box 1 and the Order No. in the ORDER NO. box.
Enter the Shipment No. in Box 2, the Date Shipped in Box 3, and the B/L and TCN in Box 4.
Enter the Discount Terms in Box 5.
Enter the Invoice No./Date in Box 6, the Page and Of in Box 7, and the Acceptance Point in Box 8.
Prime contractor and where it ships, Boxes 9 to 14
Enter the Prime Contractor Code in Box 9 and the Administered By Code in Box 10.
Enter the Shipped From (If other than 9) Code and the FOB in Box 11, and the Payment Will Be Made By Code in Box 12.
Enter the Shipped To Code in Box 13 and the Marked For Code in Box 14.
Items shipped, Boxes 15 to 20
For each item, enter the Item No. in Box 15, then the Stock/Part No. and the Description in Box 16, indicating the number of shipping containers, the type of container and the container number.
Enter the Quantity Ship/Rec'd in Box 17, the Unit in Box 18, the Unit Price in Box 19 and the Amount in Box 20.
For the * in Quantity Ship/Rec'd in Box 17: if quantity received by the Government is the same as quantity shipped, indicate by (X) mark; if different, enter actual quantity received below quantity shipped in Box 17 and encircle.
Contract Quality Assurance, Box 21
Under Box 21, Contract Quality Assurance, a. Origin or b. Destination CQA acceptance of listed items has been made by me or under my supervision and they conform to contract, except as noted herein or on supporting documents.
Receiver's Use, Box 22
In Box 22, Receiver's Use, quantities shown in column 17 were received in apparent good condition except as noted.
