This form is used by each Military Service to determine eligibility of an event for military aerial support.
Who completes it: this form must be completed by the requesting organization that is responsible for conducting the event.
Late forms: incomplete forms, or forms submitted late, cannot be considered and will be returned to the requester's representative.
Funeral honors: do not use this form to request flyovers for military funeral honors.
Demonstration teams: requests for the U.S. Navy Blue Angels and Thunderbirds must be received by July 1 of the year that is two years preceding the year of the event.
