What it is: DD Form 518, the ACCIDENT - IDENTIFICATION CARD, provides persons involved in an accident with a DoD owned/leased vehicle the identity of the person with the authority to act on the matter.
Where and when to file: When a DoD vehicle is involved in an accident, the driver provides the other party(s) with a properly executed DD Form 518.

How to fill out DD Form 518, box by box
Any correspondence regarding accident should be addressed to
Enter the address for correspondence regarding the accident under "Any correspondence regarding accident should be addressed to:".
Date of accident, make and type of vehicle, registration no.
Make reference to the date of accident, and enter the make and type of vehicle and the registration no.
Driver, SSN, grade and organization
Enter the driver as last name, first name, middle initial.
Enter the SSN, the grade and the organization.
The SSN is requested because of similarity of names, to further identify the driver of the DoD vehicle.
Disclosure is voluntary: no disciplinary action is taken in cases where the SSN is not provided.
