Aircraft and applicant
Box 1, Aircraft: enter the Make, Model, Registration Number and Serial Number.
Box 2, Applicant: enter the Name and the Address/Telephone Number.
Type of Product and Certification Basis
Box 3, Type of Product and Certification Basis: check Airframe, Engine, Appliance or Other.
Box 3, Type of Product and Certification Basis: check Part 23, Part 25, Part 27, Part 29, Part 31, Part 33, CAR 3, CAR 4(a), CAR 4(b), CAR 6, CAR 7, CAR 8 or CAR 13.
For an appliance or Other
For an appliance or Other in Box 3, list the Manufacturer, Part Number and Serial Number.
Brief Description of Project
Box 4, Brief Description of Project: enter it and check Repair or Alteration.
Schedule for Completion of Project
Box 5, Schedule for Completion of Project: enter the Date when field approval is needed, the Date when work is to begin, the Date for ASI visit (projected) and the Projected completion date for project.
Who Will Perform the Repair or Alteration
Box 6, Who Will Perform the Repair or Alteration?: enter the Mechanic's name or Repair station, the Certificate no, the Contact person at the facility and the Telephone number.
Location where alteration/repair will be accomplished
Enter the Location where alteration/repair will be accomplished.
Designees
Box 7, Designees (DARs, DERs, or ODAs): check None, or enter the Name and Telephone number for Designated Engineering Representatives (DER), Designated Airworthiness Representatives (DAR), or Organization Designation Authorization (ODA).
Compliance Statement and Previous Repairs or Alterations Affected
Box 8, Compliance Statement and Compliance Checklist: Attach the Compliance Checklist you completed.
Box 9, Previous Repairs or Alterations Affected by This Alteration: Is this alteration compatible with previously installed equipment?
Maintenance Information and Aircraft Flight Manual Supplement
Box 10, Maintenance Information and/or Instructions for Continued Airworthiness (ICA): check Yes or No for Maintenance information attached? Reference these in block 8 of FAA Form 337.
Box 11, Aircraft Flight Manual Supplement (AFMS): check Yes or No for Do you have an AFMS? If yes, attach a copy.
Data Attached
Box 12, Data Attached: check Proposed FAA Form 337, Description of alteration, including drawings, schematics, and diagrams, Material list, Processes, Specifications, Previous field approvals, FAA Form(s) 8110-3, Serviceable tags, Placards, Test data and/or flight test data, Load analysis (electrical and/or structural) or Other.

Box 13, For All the Data Submitted: check Yes, No or N/A for Are all applicable airworthiness requirements addressed? Are all exemptions addressed? Are all special conditions addressed? Are the requirements of Part 26 addressed? Are all applicable airworthiness directives addressed? Are instructions for continued airworthiness addressed? Are the applicable noise requirements addressed? Are the applicable emission requirements addressed? Are all changes to a flight manual addressed?
