How to fill out Form 2081, box by box
Effective date, employee's name, Social Security Number and driver's license
Enter the Effective Date (Month, date, year).
Enter the Employee's Name (Last, first, and middle).
Enter the Social Security Number.
Enter the State and Number of Driver's License (If applicable).
Home address, sex, height, weight, birthday and birthplace
Enter the Home Address (Number, street, apt. no., city, state, and ZIP Code).
Under Sex, enter Male or Female, then the Height and Weight.
Enter the Birthday (Month, date, year).
Enter the Birthplace (City and state).
Emergency Assignment or Permanent Assignment
Check Emergency Assignment (Not to exceed 15 days) or Permanent Assignment (Exempt from postal screening procedures).
Contractor, contract number and administrative official
The contractor states that its employee has an assignment requiring access to mail or postal premises under its contract with the United States Postal Service.
Enter the Contractor's Name.
Enter the Contract Number and the Administrative Official's Name.
