The HUD 50059 form (06/2014) is the Owner's Certification of Compliance with HUD's Tenant Eligibility and Rent Procedures from the U.S. Department of Housing and Urban Development, kept as the landlord's record of certification and not for submission to the Federal Government. The owner or agent completes it and certifies that the tenant's eligibility, rent and assistance payments were computed under HUD's regulations and administrative procedures and that all required verifications were obtained. It records summary and unit information, household members, income, assets, and allowances and rent calculations, including the total tenant payment, tenant rent and assistance payment. The head of household, spouse or co-head and other adults sign to certify that the household, income and asset information is true and complete. A continuation page is used when household members, income or asset items exceed the space allowed.
