Change Reporting: you must report changes in any source of income for anyone in your household within 10 days after the end of the month in which the change happened.
Simplified Reporting: you are only required to report changes at the time of your next recertification, except for three situations.
130% income: if your household's gross monthly income exceeds 130% of the poverty level, you must report within 10 days after the end of the calendar month.
On-line: if for some reason you can't mail, fax or bring in this form, you can also report the changes on-line through myBenefits.ny.gov.
