Form 1 of the Independent State of Papua New Guinea, the Birth and National Identity Registration Form under the Civil Registration Act (Chapter 304) Amended 2014, registers a birth, a National Identity Card, or both. The form is written in block letters, and required items are marked with an asterisk. Section A takes the child's or applicant's details: names, date and place of birth by province, district, LLG and ward, gender, registration type and type of birth. Section B takes each parent's details, place of origin, tribe and clan, residential address and the parents' marriage information. Section C, the National Identity Card information, is for applicants 18 and above only, and Section D names an authorized witness: a councillor, pastor, clan leader, health worker or professional. The applicant certifies the information, and the registration officer signs.
