The ADEM 541 form, officially titled the Scrap Tire Site Registration Form, is administered by the Alabama Department of Environmental Management (ADEM). Alabama solid waste regulations require any property owner or responsible party maintaining a site with more than 100 scrap tires to register that site with ADEM’s Solid Waste Branch.
The form collects the following required information: the property owner’s name and address, the site’s location and legal description, the estimated number of scrap tires stored, the total area the tires occupy, and the name and contact information of the party responsible for placing the tires.
After registration, ADEM’s Solid Waste Branch opens a file for the site and may provide guidance on recycling, disposal, or cleanup options. Registered site operators remain accountable for ongoing compliance with state scrap tire management rules. Electronic submission of Form 541 is accepted by ADEM.
