The AOC-E-208 form, formally titled "Application for Appointment of Guardian of a Minor," is a required court document in North Carolina. Courts use it when a minor needs a legally appointed guardian to protect their welfare or manage their estate. The form applies to three types of appointments: guardian of the estate, guardian of the person, or general guardian covering both roles.

The opening section of the AOC-E-208 establishes basic facts about the minor, including full name, date of birth, current county of residence, and domicile. It also asks whether the minor's parents are living and, if so, their current address and location. If a parent has died, the form requires the date of death and the county where any estate is being administered.

The form then asks applicants to identify all interested parties. This includes any person who has custody of the minor, a parent residing in another household, or anyone with a financial or personal interest in the outcome of the guardianship. For each party, the application requests their name, address, and relationship to the minor or to the proceeding.

A substantial portion of the AOC-E-208 covers the minor's financial situation. Applicants must list all known assets and provide an estimated value for each category. The form organizes assets into groups: cash and uncashed checks on hand, interests in real property, accounts, stocks and bonds, farm products and livestock, insurance proceeds, injury settlements, and rights of action for injury. This complete picture allows the Superior Court to set an appropriate bond amount that the guardian must maintain throughout the guardianship.

The signature section at the bottom requires each applicant to sign under oath. A notary public or another person authorized to administer oaths must witness the signatures, apply their official seal, and record their commission expiration date and the county where notarization occurred.

Once guardianship is established, ongoing court filings become mandatory. The Annual Guardian of the Person report documents the minor's physical, educational, and personal welfare each year. The Annual Guardianship Report provides an accounting of the minor's financial estate. Courts also use the AOC-E-404 form for related estate administration proceedings in the North Carolina Superior Court.
