The Ballast Water Reporting Form documents vessel compliance with the IMO Ballast Water Management (BWM) Convention, adopted in 2004. Ships carrying ballast water internationally must record each ballast water operation in detail to pass port state control inspections.
Captains and responsible officers of ships carrying ballast water must submit this form to port state control authorities. This applies to cargo vessels, tankers, and container ships on international voyages. The ship's Ballast Water Management Plan must align with data recorded here.
The form collects vessel name, type, and IMO number, voyage details, ballast water sources by port of origin, volumes discharged in cubic meters, treatment methods, and officer certifications.
