The Boiler Division of the Michigan Department of Licensing and Regulatory Affairs reviews each submission to confirm it meets the state's safety and engineering standards. • Applicant and licensed installer information — full name, Michigan contractor license number, employer name, and contact address
• Boiler specifications — manufacturer, ASME code symbol stamp, maximum design pressure (PSI), BTU/hr input, and steam capacity (lbs/hr)
• Nature and scope of work — whether the project involves installation, repair, alteration, or field erection, together with weld procedure specifications and hydrostatic test pressure data where required
• Clearance documentation — confirmation that the boiler meets minimum clearance distances, or a drawing and graph demonstrating the alternative arrangement if clearance requirements cannot be met
• Permit fee — amount varies by work type; include a check payable to the State of Michigan
