Bcia 8016Rr Request Form PDF Details

The BCIA 8016RR is a two-page document issued by the California Department of Justice. It collects your full legal name, date of birth, place of birth, gender, height, weight, eye color, hair color, Social Security number, and current address. You must select the appropriate service type: record review under Penal Code 11105.3 or foreign adoption processing under Penal Code 11105(b)(11).

A certified Live Scan operator completes the biometric portion by capturing your fingerprints electronically and transmitting them directly to the DOJ. The operator records the Originating Agency Identifier (OAI), the authorized receiving agency, and any ATI number if you are resubmitting a prior request.

Your data may be shared with the California DOJ, a foreign government embassy, or a designated agency based on your selected service type. A processing fee is paid directly to the Live Scan operator at the time of submission. If you cannot afford the fee, contact the Bureau of Criminal Information and Analysis to request a fee waiver.

The privacy notice on the form outlines your rights under the California Information Practices Act and explains how your personal data may be used or disclosed. Read this notice before signing. Your signature is required for the form to be accepted.

The BCIA 8016RR Request for Live Scan Service is a California Department of Justice form used to submit fingerprints for a personal criminal record review or a foreign adoption background check. Results are typically returned within three to five business days.
