South Africa Birth Register Insertion Form PDF Details

This is an official South Africa birth registration document used to insert a natural father's particulars in a child's birth register. Both parents must appear together under Section 11(4) of the Births and Deaths Registration Act, 1992.

Who Needs This Form

Any natural father not yet listed on a child's South Africa birth register can apply for insertion of his details. Both the natural father and natural mother must attend a Home Affairs office together and agree to the registration.

Required Documents

Both parents must bring the following to the Home Affairs office:

• Valid ID document for each parent
• The child's birth certificate showing the current register entry
• Sworn affidavit forms signed before an authorized official

Steps to Fill In the Form

The form has five parts filled in at a South Africa DHA office:

• Part A: Natural father's name, ID number, residential address, and occupation
• Part B: Additional particulars about the natural father
• Part C: Natural mother confirms the father's identity and lineage
• Part D: Sworn declaration by the natural father before an authorized official
• Part E: Sworn declaration by the natural mother before an authorized official
