California Form SC 114 PDF Details

The SC 114 form is officially titled "Request to Amend Party Name Before Hearing (Small Claims)." Use it to correct or update the name of any party listed on an existing small claims case.

You can file SC 114 only after the original claim has already been served to all parties. After filing, you must mail or personally deliver a copy to every other party in the case. The court will not send notification to other parties on your behalf.

If the original claim has not been served yet, do not use SC 114. Instead, file an amended claim using Form SC-100 (for plaintiffs) or Form SC-120 (for defendants). In some cases, you may need to start a new claim if the deadline has passed.

A judge will decide at a hearing whether to approve the name change. The outcome is not automatic. File SC 114 as early as possible before the scheduled hearing date to allow enough time for the court to process your request.
