Form PMS 245 at a glance
What it is: Form PMS 245 is the NWCG Passenger/Crew and Cargo Manifest.
Who signs: Sign on the Signature of Authorized Representative line and enter the Date.

How to fill out Form PMS 245, box by box
Crew Name, Ordering Unit and Incident/Project
Enter the Crew Name and the Flight Manager Name & Phone.
Enter the Ordering Unit and the Incident/Project Name.
Enter the Incident/Project # and the Request #.
Carrier, departure and arrival
In each row, enter the Carrier Name or Vehicle List Make/Model/License, the Departure Location, the ETD, the Arrival Location and the ETA.
Report To and If Delayed, Contact
Enter who to Report To and who to contact If Delayed.
Passenger/Cargo Name, M/F, Weight, Position/AD Class and Home Unit/Jetport
On each Passenger or Cargo line, enter the Passenger/Cargo Name (Include contact numbers for leaders) and mark M or F.
Enter the Position/AD Class (e.g., FFT2/AD-C).
Enter the Home Unit/Jetport.
Enter the Weight under Passenger and the Weight under Cargo.
# of passengers on page and Total Weight
Enter the # of passengers on page, the Weight Subtotals and the Total Weight.
Signature of Authorized Representative
Sign on the Signature of Authorized Representative line and enter the Date.
