The form is available on the Cook County Sheriff's official website. It is also distributed at divisional lobby areas where the inmate is housed. You can pick up a physical copy at the entrance of any Cook County courthouse as well.
Once you complete the form, return it to the division where the inmate is housed. CCDOC staff will review your application and notify you of the decision. Approved visitors may then schedule visits through the facility's booking system.
The form asks for the inmate's full legal name, booking number, and division. You must also provide your own full name, date of birth, residential address, phone number, and relationship to the inmate. A valid form of identification is required at the time of the visit.
Additional fields include height, gender, state of issue, and eye color. Providing accurate information is critical. Inaccurate details can result in denial of the application without appeal.
