Filling Out Cemetery and Operator Information
The first section captures identifying details about your cemetery and its operator. Enter the full legal name of the cemetery, your state-issued registration number, the reporting fiscal year, and the name and address of the authorized filing officer. These fields must match the records on file with the Illinois Secretary of State exactly.
Completing the Permanent Care Fund Section
This is the core of the annual report. Enter the fund balance at the start and end of the reporting period, total deposits made (calculated as a required percentage of lot sale revenue per 765 ILCS 835), investment income credited to the fund, and any approved withdrawals. Use your cemetery’s bank and investment statements to fill in these figures accurately.
Reporting Interments and Lot Sales
Record the number and type of interments performed during the year, including ground burials, mausoleum placements, and cremation niche placements. Also report new lot contract sales, total sales revenue, and remaining available spaces. Cemetery operators managing related documents may also need a burial claims application or an authorization for funeral arrangements.
Saving and Submitting the Completed Form
After completing all sections, use the download button to save the finished PDF to your device. Review every page before printing. The signed report must be submitted to the Illinois Office of the Comptroller by April 1 each year. Retain a copy for your cemetery’s records in case of a state audit.
