Name of the Puja Committee
Write the Name of the Puja Committee in Block Letters, the name which is written on Donation Receipt Book.
Address and e-mail ID
Write the Address and, if any, the e-mail ID.
Temporary supply, days and load
Enter the temporary supply from and to in 2026, the days and the required load in KW inside pandal.
Enter the load in KW outside pandal and the Usage of LED lights as a % of Total Load (inside load + Outside load).
Landlord of the premises where the supply will be taken
For the Landlord of the premises where the supply will be taken, a), c) and e) each have i) Name and Address, ii) Consumer No., iii) Light Post No. and iv) Load (KW).
Licensed Electrical Contractor who is supervising electrical wiring
Write the Name & Address of representative of Licensed Electrical Contractor who is supervising electrical wiring.
President, Secretary and Treasurer of the Executive Committee
Write the Name, Address & Telephone No. of A) President, B) Secretary and C) Treasurer of the Executive Committee.
Seal of the Puja Committee and signature of the Secretary
Put the SEAL OF THE PUJA COMMITTEE on the form, with the Date of the Secretary signature.
Test Form installations
In the TEST FORM, enter each Installation at the following address, 1., 2. and 3., with its Total Connected Load and Voltage.
Details of Supervisor and Details of Contractors
Under Details of Supervisor, write the Name & Address of Supervisor; under Details of Contractors, write the Name, Address and Telephone No. of Contractor.
Write the Certificate No. of Supervisor and the part of class of work for which it is enclosed, and the Registered No. of Contractor's License.
Write the Supervisor's certificate valid up to and the Contractor's certificate valid up to.
Details of workmen engaged on the work
For the workmen engaged on the work, write the No., Name, Permit No., Class of work for which the permit is valid and Valid up to.
More rules for CESC Puja temporary supply application
Warning to contractors: before submitting this form, ensure that the main switches are connected to the meter loops where provided, that the installation compiles with Regulation 35 of Central Electricity Authority (Measures Relating to Safety and Electric Supply) Regulations 2010, and that the contractor's license no., supervisor's certificate no. and workmen's permit no. are properly inserted.
Advices to the Puja Organisers: all wiring installations must be done by Govt. Licensed Electrical Contractor. Keep the puja Pandal at a safe distance of at least 2.13 meter from overhead mains.

Before you go
Fill in the line after Through: Officer (Inspector) In Charge, and write the Puja you have arranged in 2026. Submit the application duly filled in with the necessary requirement (4 in no.) i.e. No Objection from Police Station, Consent from Fire Brigade, No objection of the land owner where Puja will be organized and Test Form. You undertake to abide by the safety rules and shall not violate any statutory provisions including those related to unauthorized use/pilferage of electricity.
