The carrier liability section states the standard liability limits that apply depending on whether the shipment originates in the United States or Canada. These limits are subject to the carrier's applicable tariff. Shippers who need coverage above the stated limit must enter a declared value in the Special Agreement section and negotiate an adjusted rate with the carrier before the shipment is tendered. Hazardous material shipments require the proper DOT designation, shipping name, UN number, and packing group in the commodity description area.
A shipper certification statement at the bottom of the form requires the shipper's authorized signature and date, confirming that all listed information is accurate and that the shipment meets applicable regulations for packaging, labeling, and safe transport of the described goods.
