DA Form 4322 is the official Army Officer Candidate Contract and Service Agreement. It is required for anyone applying to attend the US Army Officer Candidate School (OCS). The form is governed by Army Regulation 350-51 (AR 350-51), which covers all rules for the OCS selection and training program.

The purpose of DA Form 4322 is to ensure that every army officer candidate fully understands their obligations before starting OCS. The Army designed this requirement to reduce high dropout rates in the officer candidate program. Candidates who sign the form have committed to their service agreement before training begins.

By signing DA Form 4322, a candidate accepts four key commitments:

• Extend their current Army enlistment period if more time is needed to finish OCS training
• Remain on active duty in their current status until all OCS requirements are completed
• Waive specific enlistment benefits and bonuses after acceptance into OCS
• Serve as a commissioned officer in the US Army after graduating from OCS

Eligibility for the OCS program, and therefore for completing DA Form 4322, extends to active duty enlisted soldiers (E-4 and above), Army Reserve members, and National Guard soldiers meeting minimum age and education requirements. Civilians who hold a bachelor's degree and meet physical fitness standards may also apply through the direct commission or reserve routes. All eligible applicants must complete DA Form 4322 as a mandatory part of their application packet.

Once a candidate signs DA Form 4322, the signed contract becomes part of the official OCS application submitted to the selection board. The board reviews the agreement to confirm the candidate understands the service commitment required. If accepted, the signed DA Form 4322 is filed in the candidate's official military training record and remains on file throughout the full OCS program. It serves as legal proof of the agreement between the candidate and the Army throughout the training process.
