DA Form 5117, the Army Reassignment Control Sheet, is the official administrative record for all U.S. Army personnel reassignments. It is governed by AR 600-8-11 and maintained under the proponent authority of the Deputy Chief of Staff, G-1 (DCS, G-1).
The form records personal data (name, Social Security Number), military identifiers (grade, PMOS, ASI, SQI), and unit assignment details. Blocks 1 through 35 are completed by the Military Personnel Division (MPD) or Personnel Service Company (PSC) to document the initial reassignment package. Blocks 36 through 40 cover follow-up actions: reenlistment or extension decisions, security clearance requirements, deletion or deferment of the reassignment, passport or visa needs, and port call scheduling.
Two copies are prepared. One is placed in the Reassigned File and one in the soldier's Military Personnel Records Jacket (MPRJ), creating a complete audit trail for the entire reassignment process.
