Who Needs DA Form 5117
This form is required for any active-duty Army soldier undergoing a formal reassignment. The unit's Military Personnel Division (MPD) or Personnel Service Company (PSC) initiates and completes the form. Individual soldiers review the completed form and retain one copy in their Military Personnel Records Jacket. The form is not completed by the soldier directly.
How to Complete DA Form 5117
The form is divided into two groups of blocks.
Blocks 1 to 35 - Initial reassignment data. These blocks record the soldier's name, Social Security Number, grade, primary military occupational specialty (PMOS), additional skill identifiers (ASI), special qualification identifier (SQI), and unit assignment. The MPD or PSC completes all blocks in this group before the soldier departs the losing unit.
Blocks 36 to 40 - Follow-up actions. These blocks are completed only when additional steps are required: reenlistment or extension processing, security clearance updates, deletion or deferment of the reassignment order, passport or visa requirements, and port call scheduling for overseas assignments.
After completion, two copies are prepared. One goes into the Reassigned File at the losing unit and the other is filed in the soldier's MPRJ.
