Purpose and Key Uses
The DD Form 515 is a Department of Defense roster used to document every person held in a military correctional facility. Corrections officers submit it regularly to the Judge Advocate office. The form provides a structured, timestamped record of custody status for each individual in the facility.
Key Fields on the Roster
Each prisoner receives one row on the form. Complete every column:
- Name and registration number – unique identifier for the prisoner
- Service branch – Army, Navy, Air Force, Marine Corps, or Coast Guard
- Sentence and minimum term – court-ordered confinement length
- Type of release – transfer, parole, discharge, or sentence completion
- Discharge date – the projected or confirmed release date
- Remarks – the authority and date for any change in custody status
Step-by-Step Instructions

- Complete one row per prisoner. Leave no field blank.
- Use only the March 2013 edition – all earlier versions are obsolete.
