The DDO Registration Form (Annexure N3) is an official government document used by Drawing and Disbursing Offices to register with India's Central Recordkeeping Agency (CRA) and begin processing National Pension System contributions for their employees.

Who Is Required to Submit the DDO Form?

The following government entities must complete the DDO Registration Form before processing any NPS contributions:

• State Government Departments and Ministries responsible for employee payroll
• Union Territory Administrations including offices in Delhi, Chandigarh, and Puducherry
• State Autonomous Bodies (SABs) whose employees are required to enroll in NPS
• Any Drawing and Disbursing Office managing salary disbursement for more than 10 NPS subscribers

Central Government offices use a separate registration process and do not need to complete Annexure N3.

What Information Does the DDO Form Collect?

Completing this registration form accurately requires the following information:

• DDO type: State Government, Union Territory, or State Autonomous Body
• Full legal name and complete address of the Drawing and Disbursing Office
• Official phone number and email ID of the DDO
• Name and designation of the authorized signatory
• Tax Deduction and Collection Account Number (TAN) if applicable
• District Treasury Office (DTO) name for authorization

All mandatory fields must be completed in block letters. Any corrections to the form require initials from the authorized signatory.

How Is the DDO Registration Form Submitted?

After completing all fields, the DDO Form must go through a formal authorization process:

1. The head of the Drawing and Disbursing Office signs and stamps the completed form
2. The form is forwarded to the District Treasury Office (DTO) for review and counter-authorization
3. The DTO affixes their official stamp and signature in the designated section
4. The authorized form is submitted to the Central Recordkeeping Agency for processing

Forms submitted without valid DTO authorization, or with incomplete mandatory fields, will be rejected. Make sure all details match official government records to avoid processing delays.

What Happens After Registration Is Complete?

Once the CRA processes the submission, the Drawing and Disbursing Office receives a unique DDO Registration Number. This number serves as the official identifier for all NPS transactions, including monthly contribution uploads, correction requests, and subscriber account transfers. Keep a copy of the registration form and CRA confirmation for official records.
