Check the next box if you understand that if you want to withdraw your consent at any time before the minor applicant reaches the age of 18, Penndot will cancel their driver's license.
Check the next box if you understand you will be required to ensure the applicant will have to complete at least 65 hours of supervised behind-the-wheel skill-building before they will be permitted to take the driver's test. The skill-building includes no less than ten hours of nighttime driving and five (5) hours of bad weather driving. The supervising adult must be at least 21 years of age or older.
Check the last box if you understand Penndot recommends the supervising adult refer to the Parent's Supervised driving Program guidebook when teaching the minor applicant how to drive.
Organ donor designation
