Evidence: this certification will be accepted as one form of evidence for your principal residence address.
Without a lease: the property owner certifies the above-named individual maintains a primary residence at the owner's home without a rental or lease agreement.
Lease: the owner certifies the individual has maintained a primary residence on the owner's property for at least 60 days and entered into a rental/lease agreement.
Social Security Number: you are required to submit it so that the state may administer laws related to licensing drivers. It is not required for a DAC.
