What it is: VP-247 is the Registration Services Transaction Request: if more than one vehicle is requested for a client a transaction request must be submitted for each vehicle.
Who signs: The Authorized Representative enters the Printed Name, Signature and Phone #.

How to fill out Form VP-247, box by box
Business Name and SOS NVDP #
Enter the Business Name, the SOS NVDP # and its Exp.
Requesting (check transaction(s))
Under Requesting (check transaction(s)), mark Title, Registration, Temp Movement Permit, COA or Other, and enter the Online Portal or Kiosk Error Code.
Owner's Full Legal Name
If there are more than two owners, complete and attach an additional Registration Services Transaction Request form.
For each owner, enter the Owner's Full Legal Name as First, Middle and Last.
For each owner, enter the NV Driver's License, Identification, DAC Number or Date of Birth. The D.O.B. is only needed if never issued NV DL, ID, or DAC.
For each owner, enter the FEIN # for Fleet Insurance only.
For each owner, enter the Physical Address and the Mailing Address, each with Address, City, State and Zip Code.
Customer's Telephone Number and Email
Enter the Customer's Telephone Number and the Customer's Email (optional).
Vehicle Identification Number and Payment Submitted
Enter the Vehicle Identification Number. Under Payment Submitted, mark Cash, Check, Credit Card or ACH.
Year, Body Type, Model and Make
Enter the Year, Body Type, Model, Make, Fuel, Unladen Weight (Trailer), Length, County based in, Axles, Declared Gross Weight (Truck) and Odometer (required).
Title and Registration Documents
Under Title and Registration Documents (check all that apply), mark Title or MSO, Purchase Order, Lease Agreement, NV Renewal card, Emission Certificate, Bill of Sale, EDRS or Lien Sale, VIN Inspection and NV Insurance card. Mark Exemption #, Erasure Affidavit, Nevada LIVE or Other. Mark Application for Registration (VP-222), Out of State Registration (State and License Plate #), Personalized Plate application or Temporary Movement Permit, and enter the days requested and the reason. Ownership documents must be marked in the Title and Registration Documents section. License Plate Information - must be provided if available. Note: Use any/all credit (linked to customer) available in system at the time of processing registration: mark Yes or No. Mark Transferring Plate or Surrendering Plate, and for Surrendering Plate # enter the plate number. If box is not marked - refund will not be processed.
Mark New Plate, or Specialty Plate Type, or Trailer Plate Small or Large, and Trailer Registration 1yr or 3 yrs.
