California dealers, autobrokers, and other DMV-licensed firms used the REG 397 Order Form to request printed REG 397 New Report of Sale from the DMV’s Occupational Licensing Section. The form ties each order to the firm’s legal identity, license number, and past usage so DMV can control how many secure documents are issued.
Form OL 395N asks for the dealer’s firm name, firm number, business and mailing addresses, the number of sheets requested, and a 12-month history of the first and last New Report of Sale numbers issued and used. The signer, who must be a sole owner, partner, corporate officer, or managing member, certifies under penalty of perjury that the information is correct.
DMV staff complete the lower section when issuing new REG 397 books or sheets.
Note: Starting in 2019, California law (AB 516) moved Reports of Sale to an electronic ROS (eROS) system. Dealers now get ROS numbers online through approved providers and print the ROS and temp tags on demand.
