Attorney or party without attorney
Enter the Attorney or Party Without Attorney (Name, State Bar number, and address). Do not use FOR COURT USE ONLY.
Enter the Telephone No., the Fax No. (Optional) and the E-mail Address (Optional).
Enter the Name after Attorney For.
Superior Court of California, County of, and case
Enter the county of the Superior Court of California, the Street Address, the Mailing Address, the City and Zip Code and the Branch Name.
Enter the Case Name and the Case Number.

You must attend court or provide to the court the documents listed in the order. Follow the orders checked in item 2. If you do not, the judge can fine you, send you to jail, or issue a warrant for your arrest.
In 1, enter the name or business after To.
Attend the hearing (item 2, a and b)
Box a is Attend the hearing.
Box b is Attend the hearing and bring all items checked in c.
Provide a copy of these items (item 2c)
Box c is Provide a copy of these items to the court (Do not use this form to obtain Juvenile Court records), with lines (1), (2) and (3) for the items.
If the box after line (3) is checked, provide all items listed on the attached sheet labeled “Provide These Items.”
Custodian of Records and delivery within 5 days (items 2d and 2e)
Box d: If someone else is responsible for maintaining the items checked in c, that person (the Custodian of Records) must also attend the hearing.
Box e: If this box is checked and you deliver all items listed in c to the court within 5 days of service of this order, you do not have to attend court if you follow the instructions in item 5.
Court hearing date, time and court
In 3, Court Hearing Date, enter the Date, Time, Dept. and Rm., and the name and address of the court where the court hearing will be.
Call the person listed in item 4 to make sure the hearing date has not changed. If you cannot go to court on this date, you must get permission from the person in item 4.
You may be entitled to witness fees, mileage, or both, in the discretion of the court. Ask the person in item 4 after your appearance.
The person who has required you to attend court or provide documents
In 4, the person who has required you to attend court or provide documents, enter the Name, Phone No. and Address (Number, Street, Apt. No.), City, State and Zip.
Date, signature, name and title
Under SIGN, enter the Date, sign the Signature line and enter the Name and Title.

Put all items checked in item 2c and your completed Declaration of Custodian of Records form in an envelope. You can ask the person in item 4 where to get this form.
Attach a copy of page 1 of this order to the envelope, put the envelope inside another envelope, then attach a copy of page 1 of this form to the outer envelope or write this information on the outer envelope: the Case name, the Case number, your name and the hearing date, time, and department.
If box e is checked and you deliver all items listed in c to the court within 5 days of service of this order, you do not have to attend court if you follow the instructions in item 5. To do so, seal and mail the envelope to the Court Clerk at the address listed in item 3 or the court address in the caption on page 1.
You must mail these documents to the court within five days of service of this order.
If you are the Custodian of Records, you must also mail the person in item 4 a copy of your completed Declaration of Custodian of Records. Do not include a copy of the documents.
