DD Form 1840 (AUG 2026), Joint Statement of Loss or Damage at Delivery, records the condition of household goods when a Service Provider (SP) delivers a Department of Defense shipment. The SP completes Section II: the customer's name, DoD ID, rank, origin and destination, order number, pickup date, code of service, net weight and the SP's company details. In Section III the customer acknowledges receipt and whether unpacking and debris removal were provided, and both the customer and the SP's representative sign. Section IV lists each missing or damaged item by inventory number, or reads NONE, and both sides sign it before the SP leaves. The form is notice, not a claim: written notice of further loss or damage is due within 180 days of delivery, a claim filed with the SP within nine months of delivery keeps eligibility for FRV, and claims may be filed for up to two years with eligibility only for FMV (depreciated value).
