Form 4506-C, IVES Request for Transcript of Tax Return, asks the IRS to send tax transcripts to a third party through the Income Verification Express Service (IVES). Lines 1 and 2 identify the taxpayer and, for a joint return, the spouse, by name and taxpayer identification number. Lines 3 and 4 give the current address and any previous address shown on the last return filed. Line 5 names the IVES participant, with its ID number and SOR mailbox ID, and the client's name, telephone number and address. The form notes that the transcript is sent to the third party entered on line 5a or 5d. It warns not to sign the form unless all applicable lines are completed, and says a request may be rejected if the form is incomplete or illegible.
