Due to an increase in fraudulent acts associated with the unnumbered CERTIFICATE FOR INTERSTATE MOVEMENT OF SMALL ANIMALS (CDFA Form 77-043), the California Department of Food and Agriculture no longer provides Form 77-043 for use.

Effective January 1, 2011, the UNITED STATES INTERSTATE AND INTERNATIONAL CERTIFICATE OF HEALTH EXAMINATION FOR SMALL ANIMALS (APHIS Form 7001) is the small animal health certificate for use by California accredited veterinarians. To obtain an order of the APHIS Form 7001, FAX, E-mail, or mail an Order Request to:

FOR ACCREDITED VETERINARIANS

A.Resources for Animal Movement Requirements

1.Obtain information for interstate movement requirements for animals from:

3.Obtain airline-specific requirements from the airline carrier to be used for movement of small animals by air. Some airline carriers have additional required statements of health for inclusion on the Interstate and International Certificate of Health Examination for Small Animals (APHIS Form 7001).

B. Responsibilities of Accredited Veterinarians Preparing Certificates of Veterinary Inspection

1.Certificates of Veterinary Inspection (CDFA Form 77-010) and Certificates of Health Examination for Small Animals (APHIS Form 7001):

Must be completed in compliance with federal requirements (9 CFR Part 161)

Must comply with the requirements of the state of destination and airline if shipment is by air

Must be complete and legible.

2.Distribution of Completed Certificates

U.S. Interstate and International Certificate of Health Examination for Small Animals (APHIS Form 7001)

For interstate movement, distribute the Parts 1-6 of the Form 7001 to the recipient designated in RED on the lower right hand corner of each part.

Note: Send Part 4 to the State Veterinarian in the State of Destination. For international movement of small animals from the United States, the completed and signed U.S. Interstate and International Certificate of Health

Examination for Small Animals (Form 7001) must be endorsed by a USDA APHIS

Veterinary Services area office to be valid.

Certificates are valid for 30 days after the date of animal inspection.

California Certificates of Veterinary Inspection (Form 77-010) for Livestock and Poultry

1.Distribute the form parts as indicated on the form

2.Send White and Yellow pages of completed certificates to AHB Headquarters within 10 days of examination.

3.Certificates are valid for 30 days after the date of animal inspection.
