Use this form if you delivered to the tenant a demand for possession for nonpayment of rent, and at least 7 days have passed since the date you delivered the demand for possession.
Plaintiff states: a copy of the lease or occupancy agreement, if any, under which possession is claimed, and a copy of the demand for possession is attached.
Jury trial (note to plaintiff): if you wish to demand a jury trial, you must file a jury demand (MC 22) with the complaint.
Residential property: the plaintiff's declaration that the property was kept fit for the use intended must be checked if the property is residential property.
Rent items: rental rate, payable on, rent is paid through, total rent due now, other money is due.
