Form MC-356 at a glance
What it is: Form MC-356 is the Acknowledgment of Receipt of Order and Funds for Deposit in Blocked Account.
Who signs: "I certify that the foregoing information is true and correct, that I am authorized to execute this acknowledgment of receipt on behalf of the depository named in 4, and that no withdrawal of principal or interest from this account will be permitted without a signed, file-stamped order under this case name and number."

How to fill out Form MC-356, box by box
Attorney or party without attorney
Under Attorney or party without attorney, enter the State bar number, Name, Firm name, Street address, City, State, Zip code, Telephone no., Fax no., Email address and Attorney for (Name).
Court and case
Enter the county after Superior Court of California, County of, then the court's Street address, Mailing address, City and zip code and Branch name.
Enter the Case name and the Case number.
Name and title on account and the depository
In 3, enter the name and title on account.
In 4, enter the name of depository, then in a. the branch and in b. the address.
Account number, date opened and amounts
In 5, enter the account number.
In 6, enter the date account opened.
In 7, enter the amount of initial deposit.
In 8, enter the current balance.
Date, signature and title
Enter the Date, sign on the Authorized signature line, and enter the Type or print name and the Title.

You acknowledge receipt of the Order to Deposit Funds in Blocked Account (form MC-355), a copy of which is attached to this form, and of the funds specified in item 7, below.
The account, in which funds have been deposited under the court's order, is an interest-bearing, federally insured blocked account.
