LICENSE FEE REFUND- If one of the reasons listed below applies to you, provide the information requested in Step 3 on page 2, sign the Certification, and return the form to DMV with any additional documents that are required.

♦Youcanprovideacceptable prooftosupportthatyoupaidforthelicensetwiceoryoupaidthewrongfee; or

♦Youcanprovethatyoupaida$25.00or$50.00licensesuspensionterminationfeeinerror;or

♦Youpaidforarestricteduselicense,andthesuspensionorrevocationonwhichitwasbasedwasrescinded;or

♦Youturnedina NewYorkStatedriverlicensethat isvalidfor8years,that isNOTCURRENTLYsuspendedorrevoked,andthathasan "Expires"dateonitthatismorethan4years(48months)afterthedateyouturnedinthelicense; or

♦Youappliedforalicensefromanotherstate,andyour8-yearNewYorkStatedriverlicensedoesnotexpireforatleast4moreyears(48months).

♦Youpaidacivilpenaltyfordrivingwithoutinsurance/uninsuredaccident andtherevocationonwhichitwasbasedwasrescinded.

IMPORTANT: Ifyouhavenotturnedinyour8-yearNewYorkStatedriverlicense,attachit tothiscompletedform.Ifyouhaveadriver licensefromanotherstate,attachaphotocopyofyournewlicensetothiscompletedform.

NOTE: Refundsareonlyavailableforlicensesthatareissuedfor8years.Youcannotapplyforarefundofafeethatpaidfora driver license thatisvalidforanyperiodoftimelessthan8years(forexample,a licensethat isvalidforonly4or5years).Ifyouareeligible forarefundaccordingtotheconditionsabove,youwillreceivearefundequaltotheamount youpaidfortheremaining4yearsofthe

REGISTRATION FEE OR CIVIL PENALTY REFUND- If one of the reasons listed below applies to you, provide the information requested in Step 3 on page 2, sign the Certification, and return the form to DMV with any additional documents that are required.

♦Yourequestedcredit:whenyouturnedinyourplates,youtoldstaffintheMotorVehiclesofficethatyouplannedtousetheregistration again.However,youwillnotusetheregistration,whichisvalidfor1or2fullyears.Youmustapplyfortherefundnolaterthan14months afterthedatetheregistrationwasissuedtoyou;or

♦Theregistrantisdeceasedandtherefundcheckmust bemadepayabletothenextofkin.CompleteformAC-934fromtheOfficeofthe State Comptrollerandattachitwithaphotocopyofthedeathcertificatetothisform; or

♦Theregistrantisdeceasedandtherefundcheckmust bemadepayable"totheestateof"the deceasedregistrant.Attachaphotocopyofthe DeathCertificate (CertificateofDeathorCertificationofDeath)tothisform.

♦Youpaidthewrongregistrationfee,orpaidtwice;or

♦Youreceivedcustomplatesandreturnedthemunused,oryoucancelledacustom platesorder;or

♦Youcanprovethatyoupaidacivilpenaltyforaninsurancelapsesuspensioninerror;or

♦Youenteredmilitaryserviceafteryouregisteredyourvehicle.Youwillnot use theregistrationandtheregistrationisnotrevokedor suspended.Entryintomilitaryservicemustbedocumentedbyyourcommandingofficer;or

♦YousurrenderedyourapportionedplatesandunusedregistrationtotheIRPofficewithin2monthsafterthedateofpurchaseorwithin2months afterthebeginningoftheregistrationperiod.
