The UCS-5 form (revised 10/2023) is the Application for Employment of the State of New York Unified Court System, an Equal Opportunity Employer. The employment eligibility section asks about United States citizenship or the legal right to work, and the legal history questions cover convictions, pending criminal charges, dismissals from employment, discharges from the armed forces that were other than honorable, and whether the applicant is currently in violation of a court order for child or spousal support.
