DLA Form 2500 (NOV 2022), Certification of Information Technology Disposition, replaces all similar forms, such as the older DLIS Form 1867 hard drive certificate. Each sheet carries four identical certificates, one per drive. A certificate records the drive's serial number, barcode number and make or model, and whether its storage components were removed. It certifies that the drive was cleared, purged or destroyed in accordance with DoD I 8500.01, DoD M 4160.21 Vol 4 and NIST SP 800-88 Rev 1, and records the method used (degauss, overwrite, block erase, crypto erase or other), the date, the software or degausser (manufacturer, product version and date) and the method of destruction, such as an approved metal destruction facility. The person certifying gives a printed name, signature and date, rank or grade, phone, email, organization and unit name, and a DTID or hand receipt number.
