IRCC IMM 1436 Request to Amend COPR (IMM 5292)

In Part A, Personal Details, enter 1 - Client ID number/UCI; 2 - Surname(s) (Family name) and 3 - Given name(s) as they appear on your document to be amended; 4 - Other Surname(s) (Family name) used and 5 - Other given name(s) used.

Gender/Sex: F Female, M Male or X Another gender, as it appears on your document to be amended, or, if not on the document, as it appears on your passport.

Date of birth (YYYY-MM-DD): as it appears on your document to be amended.

Place of birth (City/town and jurisdiction): as it appears on your document to be amended, or, if not on the document, as it appears on your passport.

Country of birth and Citizenship: as it appears on your document to be amended.

Passport: Passport country of issuance, Passport number, indicate if On entry or Current, Passport date of issue (YYYY-MM-DD) and Passport expiry date (YYYY-MM-DD).

Current marital status: Never married, Married, Common-law partner, Widowed, Separated or Divorced.

Marital status on entry: as it appears on your document to be amended, if applicable.

Language of correspondence: English or French.

If applying to amend a temporary resident document, you must provide a Canadian mailing address.

Current mailing address: P.O. box, Apt./Unit, Street no., Street name, City/Town, District (if applicable), Jurisdiction (Province/Territory/State), Postal code and Country.

Is the residential address the same as the mailing address? No or Yes.

20 - Telephone no. and 21 - Alternate Telephone no.: Type, Country Code, No. and Ext.

Indicating an e-mail address will authorize all correspondence, including file and personal information, to be sent to the e-mail address you specify.

Date of original entry (YYYY-MM-DD) and Date permanent residence or protected person status was granted, or date temporary resident document to be amended was issued (YYYY-MM-DD).

Part B, Information to be Amended, 1 - Indicate for which document you need a correction: Confirmation of Permanent Residence (or Record of Landing), Temporary Resident Permit (TRP), Protected Person Document or Temporary Resident Document: Permit to Come Into or Remain in Canada, Work Permit, Study Permit or Visitor Record, each with Doc No. OR Issue date and Expiry date. If you have more than one valid temporary resident document, indicate the document ID number or if unknown, the document issue date and expiry date of the requested document.

In Part B, Information to be Amended, only fill in the boxes that require a correction.

If you are applying for an amendment to your Confirmation of Permanent Residence (or Record of Landing) and you are not a Canadian citizen, have you, since your admission to Canada as a permanent resident, been convicted of a crime or offence in Canada or elsewhere? Original Request to Amend Valid Temporary Resident Documents, or Information contained in the Confirmation of Permanent Residence or Protected Person document completed and signed.

Photocopy of the passport you provided with your original application OR photocopy of your current passport OR photocopy of federal or provincial/territorial government issued photo identification OR if unavailable, photocopy of government issued or internationally recognized photo identification from outside Canada prior to your entry to Canada.

Photocopy of another form of government issued or internationally recognized identification from outside Canada before your entry to Canada indicating an error was made.

Photocopy of other supporting documents (letter of acceptance or enrolment from your current designated learning institution; Labour Market Impact Assessment or exemption information from your employer; any legal documents showing name or date of birth changes if you have obtained a legal name change or date of birth correction) if applicable.

Copies of court documents if you have been convicted of a crime since becoming a permanent resident (if you are not a Canadian citizen).

Original Use of a Representative (IMM 5476) form completed and signed, if applicable.

Proof of urgency, if applicable.

Immigration document containing error: photocopy of the erroneous Work Permit, Study Permit, Visitor Record or Temporary Resident Permit (IMM 1442). Or photocopy of the Record of Landing (IMM 1000), Confirmation of Permanent Residence (IMM 5292, IMM 5509 or IMM 5688), Permit to Come Into or Remain in Canada (IMM 1263), or document issued to a Protected Person (IMM 5292 or IMM 5715).

If you are requesting a name or gender change that was not an administrative error, you must provide the following documents.

Original application form Request for a Change of Name on IRCC Documents (IRM 0001) completed and signed (for name change).

Original application form Request for a Change of Sex or Gender Identifier (IRM 0002) completed and signed (for gender change).

Photocopy of the fee receipt showing the amount paid (See the "Pay the Fees" section in the Instruction Guide for the correct Name and/or Gender Change fee).

Photocopy of the linking document(s) that tie the previous name to the new name (for name change requests).
